This case highlights why a customized anatomical approach is vital for lower facial rejuvenation, even in patients at their ideal body weight. My patient is a young female who was unhappy with her genetic neck contour and lack of jawline definition.
Upon clinical evaluation, I determined that standard liposuction alone would be insufficient. The culprit behind her submental fullness was located beneath the muscle layer—specifically, subplatysmal fat. To address this, I recommended deep neck contouring performed through a discreet submental incision, combined with VASER liposuction to seamlessly smooth and sculpt the superficial subcutaneous layer.
She is shown here at over 3 months post-op, maintaining a stable weight with an excellent result. Her jawline is sharply defined, and the heavy submental angle has been completely transformed into a graceful, youthful contour.
Could I have achieved a similar profile with standard liposuction alone? Perhaps temporarily. However, relying solely on liposuction to fix deep fullness forces the surgeon to over-resect superficial fat. This aggressive thinning creates a high risk of premature skin aging, irregular contours, and early visibility of the platysmal bands as the patient ages. By addressing the deep anatomy directly, I achieved a superior, structurally sound, and long-lasting result.
