Patient presented with a ruptured left saline breast implant and capsular contracture of the right breast for a couple years. She had previous a breast augmentation and then a lift many years ago. She was concerned about her asymmetry and loss of volume and drop of the breast tissue. We discussed her goals and recommended removal and replacement of her saline implants to shaped silicone implants. We also used her existing capsular tissue to provide an internal bra by using a capsular flap in a new subpectoral pocket and revised her breast lift scars. This is a powerful technique Dr. Levesque learned during fellowship with Dr. Grotting and wrote a chapter in the breast repair manual textbook detailing the technique. She is shown after her surgery with good positioning of her implants and overall good shape.
