Our patient sought a natural improvement around the eyes without looking overdone. On evaluation, we noted brow descent, asymmetry, and laxity at the lower lid to cheek junction with excess skin and fat. While there was extra skin on the upper lids, we deferred upper eyelid surgery to avoid an "operated-on" appearance.
We performed a direct brow lift, placing the incision at the top of the brow hairs. Given his strong brow hair density, this approach offered excellent lift and symmetry correction with a well-camouflaged scar. We also performed a transcutaneous lower lid blepharoplasty to remove excess skin, tighten the muscle, and reduce fat in the lower eyelid area.
This combination of male brow lift and lower eyelid surgery was done under local anesthesia with oral sedation, avoiding general anesthesia. Recovery for the brow was quick, while the lower lids took about 2–3 weeks to appear socially appropriate.
Shown just over three months post-op, the patient has a refreshed, more open appearance while maintaining a natural masculine look. His results are consistent with our approach to male plastic surgery—refined, balanced, and discreet. Scars are inconspicuous, and no intravenous sedation was needed.
