Our patient presented with concerns about the appearance of her lower eyelids, specifically fullness, skin laxity, and contour irregularities that contributed to a tired, aged look. On examination, we identified excess lower eyelid fat, redundant skin, and mild laxity of the lower lid support structures. Additionally, early descent of the midface was contributing to increased visibility of the orbital rim and loss of smooth lid-cheek transition.
To address these concerns, we performed a subciliary lower eyelid blepharoplasty combined with lateral canthopexy to support and stabilize the lower lid. We also incorporated autologous fat transfer to the midface to restore volume, improve the lid-cheek junction, and create a more youthful contour.
At over four months post-procedure, she demonstrates a natural, refreshed result. The under-eye area appears smoother and more youthful, with improved contour and support. Her overall appearance is brighter and more rested—without looking overdone.
