This case involves a man in his 30s, physically fit with very low body fat, who was concerned about fullness under his chin. After an evaluation using ultrasound and physical examination, we identified the issue as deeper fat located beneath the platysma muscle and above the deep digastric muscles. Unlike subcutaneous fat, this deeper fat cannot be addressed with liposuction, Kybella, or other non-surgical fat reduction methods.
We recommended a deep necklift or anterior open necklift, a specialized procedure involving precise excision of this deeper fat and tightening of the anterior platysma. The procedure was performed in-office under local anesthesia with light sedation, ensuring a safe and comfortable experience.
The results were subtle yet impactful: a more defined jawline and tighter neck contour with no visible scars, irregularities, or an overly “done” appearance. The patient looks natural—like himself but with enhanced neck definition.
This case highlights the personalized nature of deep neck contouring. For younger patients with minimal laxity and smaller amounts of excess fat, the changes are refined but significant. For older patients with more skin and muscle laxity, the transformation can be more dramatic.
