Our patient, a man in his 70s who is in great overall shape, had lost significant weight due to lifestyle changes and felt his appearance did not match how he felt. His primary concern was his lower face and neck. After a thorough examination and full history, we determined that an anterior approach to the neck would be most effective. We chose a T-Z Necklift to remove excess skin and tighten the platysma muscle, significantly improving the submental region. I perform the T-Z Necklift in my office under local anesthesia, making it a safer procedure compared to those performed under intravenous sedation or general anesthesia. Although this approach results in a visible scar from the hyoid region to the thyroid cartilage, the scar is quite acceptable in men with thick skin and beard follicles, given the substantial improvement in contour. Early post-operative photos illustrate the scar's discreet location in natural creases under the chin and at the thyroid level. He looks fantastic, and we are honored to have helped him achieve his aesthetic goals.
