Mommy Makeover for Women Over 40: What to Expect and How It’s Different

More and more women in their 40s and beyond are choosing to undergo a mommy makeover—a combination of plastic surgery procedures that help restore the body after pregnancy and aging. At Levesque Plastic Surgery in Austin, we understand that cosmetic needs change with age, and that a woman in her 40s often has different goals, skin characteristics, and recovery expectations than someone in her 20s or 30s.

In this guide, we cover how mommy makeovers are uniquely tailored for women in their 40s and why this stage of life can be an ideal time to finally prioritize yourself.

What Is a Mommy Makeover?

A mommy makeover typically includes a tummy tuck, breast lift (with or without implants), and liposuction. In some cases, procedures like labiaplasty, arm lift, or non-surgical treatments like Morpheus8 or CO2 laser resurfacing are also added to rejuvenate skin or address intimate concerns.

Rather than focusing on a single area, a mommy makeover treats multiple zones of the body that are commonly affected by pregnancy, weight fluctuations, or the natural aging process.

Why Women in Their 40s Are Ideal Candidates

There’s a misconception that cosmetic surgery is only for women in their 20s or 30s. In reality, your 40s might be the best time for a mommy makeover.

  • You’re likely finished having children, which means your results won’t be reversed by future pregnancies.
  • Your children may be in primary or secondary school and able to care for themselves with a little support.
  • You’re more emotionally and financially prepared, and often have a better understanding of what you want.
  • You’ve prioritized your family—now it’s time to focus on yourself.
  • If not now then when?

If you’re noticing sagging breasts, stretched abdominal skin, stubborn fat, or changes in your confidence post-pregnancy or after major life transitions, a mommy makeover may be right for you.

How Is a Mommy Makeover Different for Women Over 40?

While the procedures may sound the same, women over 40 have unique medical, hormonal, and cosmetic needs that shape how we plan and execute your surgery.

  • Skin Elasticity - As we age, collagen production slows, and skin elasticity decreases. This means procedures like a breast lift or tummy tuck may require more extensive skin removal and muscle tightening to achieve smooth, firm results.
  • Hormonal Changes - Perimenopause or early menopause may contribute to weight distribution changes, thinner skin, or slower healing. Our team considers hormone-related changes when customizing your surgical plan.
  • Health Considerations - In your 40s, it’s especially important to assess: Blood pressure, Blood sugar, Cardiovascular health, Thyroid function, and Smoking history
  • Be sure to look at other moms in your age range undergoing mommy makeovers compared to younger moms or younger women in general.  Be wary of comparing yourself to Instagram and TikTok influencers as most of what is seen on social media is not real.  There are tricks with posing, lighting, and downright photoshopping images.

At Levesque Plastic Surgery, we carefully review your medical history to ensure the safest, most successful outcome possible.  Part of that may require labwork such as a complete blood count, complete metabolic panel, and with history of cardiovascular conditions perhaps an EKG or notes from any cardiologists you may have seen.  Each of our patient will be evaluated prior to just ordering labwork or requiring EKGs, Chest x-rays etc.

Popular Procedures in a Mommy Makeover After 40

At Levesque Plastic Surgery, we customize each mommy makeover based on your anatomy, goals, and lifestyle. Here are the most common procedures chosen by women in their 40s:

  • Tummy Tuck (Abdominoplasty) - Pregnancy, weight changes, and age can lead to loose abdominal skin, stretch marks, and diastasis recti—a separation of abdominal muscles. A tummy tuck contours the abdomen by removing excess skin, tightening muscles, and reshaping the waist. For women over 40, we often perform a full abdominoplasty to address the entire abdomen, and many times combine it with liposuction to the flanks or hips for more defined results.

  • Breast Lift (Mastopexy) With or Without Implants - Sagging breasts (ptosis) are common in the 40s due to gravity, breastfeeding, and loss of skin elasticity. A breast lift reshapes and repositions the breast tissue and nipple to a more youthful position.

You may choose to:

  • Lift only (if you’re happy with your current volume)
  • Add implants (if you want more fullness or upper pole projection)
  • Replace or remove old implants (if you’ve had previous augmentation)
  • Sometimes adding your own fat to the breasts with fat grafting is possible to enhance volume or thicken the tissues.

  • Liposuction - Even with a healthy diet and regular exercise, hormonal changes in your 40s may cause fat to accumulate in the: Flanks (“love handles”), Lower abdomen, Back or bra roll, Thighs, arm, etc. Liposuction is frequently added to a mommy makeover to contour these areas and enhance your silhouette.  The combination of liposuction and an abdominoplasty called lipoabdominoplasty is now the standard of care.  Something I have been doing for over 15 years and with further training in high definition liposuction with Dr. Hoyos in Columbia  we not only flatten the tummy during tummy tuck we also contour the fat to enhance musculature.  The leaves a three dimensional abdomen that has gentle s curves that we naturally see in women.  A totally flat tummy is actually a very artificial type appearance.

Optional Enhancements

Women over 40 may also choose to include:

  • Labiaplasty (to improve comfort and confidence)
  • Arm or thigh lift (especially after weight loss)
  • Skin tightening or resurfacing (like CO2 laser or RF microneedling)
  • Non surgical enhancements such as Botox or Fillers.
  • Add Renuvuion  for additional skin tightening.

We’ll help you select the most effective combination for your goals during your consultation.

Customizing Your Mommy Makeover Plan

No two women are alike, especially in their 40s. Your mommy makeover should be as unique as your life story.

At Levesque Plastic Surgery, we will:

  • Evaluate your skin, muscle tone, and goals
  • Review your medical history and hormone factors
  • Create a personalized surgical plan that safely addresses your aesthetic concerns

Most times we can perform all of your desired procedures in one step.  However we are not afraid to insist on safety and break up your transformation into two or more steps when indicated.  It's always better to avoid a possible complication and remain safe rather than trying to do too much.

We also coordinate post-operative care, whether you need in-home nursing, lymphatic massage, or just help planning childcare and recovery time as necessary.

Recovery Expectations for Women Over 40

Your recovery may look slightly different at 40+ than it would have in your 20s. That doesn’t mean it’s harder—it just requires smart planning and self-awareness.

Recovery Timeline

  • Week 1–2: Rest, light movement, pain management, support garments - this is the time where having some additional help with home tasks and especially young children is crucial.
  • Week 3–4: Gradual return to desk work, light household tasks - most moms have returned to the usual tasks but still take extra time to get things done.  This is not the time to hit the gym hard to make up for lost time.
  • Week 5–6+: Resume exercise, see final results begin to take shape - here is where patience on the process is needed.  There is still swelling, results are not final.  While most patients will have such a remarkable change that even at this point mom's are ecstatic at the results it will be a good year before healing is near complete and the results will speak for themselves.  Now is the time to commit to maintaining your results by committing to a healthy diet and regular exercise.

Tips for a Smooth Recovery

  • Eat nutrient-dense, anti-inflammatory foods
  • Hydrate and get sleep—your body needs it!
  • Consider compression therapy or post-op massage
  • Compression garments are important to control swelling.  I personally don't like too much right away but once you can get into and out of a faja on your own than it's go time.  Lipo foam, KT tape can sometimes be recommended to maximize results but we go through that in the post op visits.
  • Get help with family duties so you can truly rest - most moms do so much around the household that a real good assessment of who else in the household can manage those tasks is critical to a smooth recovery.

We provide detailed instructions and ongoing support throughout your healing process.

Realistic Results: What to Expect in Your 40s

While some younger patients want dramatic changes, many women in their 40s are looking for natural, proportional results—not to look “done,” but to look like the best version of themselves.  We'd love for you to feel confident to rock the skimpiest little bikini you desire but many moms are just as happy to be in a nice one piece, our job is to give you a result that you can do either.

Expect:

  • A flatter, firmer abdomen
  • Perkier, more youthful breasts
  • Better fitting clothes and improved posture
  • Renewed confidence in and out of your wardrobe

Other benefits many patients will have are improvements in lower back pain and stress incontinence which is a result of repositioning of the rectus muscles and a stronger abdominal core.  These effects are never guaranteed but we see them enough to know improvements are certainly possible.

Results typically continue improving for a whole year post-op and can last many years with a healthy lifestyle.

FAQs: Mommy Makeover Over 40

Q: Is it safe to combine procedures at my age?

A: Yes, for healthy patients in their 40s, combining procedures like a tummy tuck and breast lift is generally safe. We’ll perform a full medical assessment beforehand to reduce any risks.  We know that increasing time under anesthesia does increase the risk of complications and the number of procedures that can be safely combined is discussed and customized for each patient.

Q: Will my hormone levels affect surgery or recovery?

A: Possibly. We account for estrogen, collagen changes, and metabolism when planning your procedure and recovery. Let us know if you’re on HRT or approaching menopause.  We also remove, permanently, a percentage of fat cells in you body depending on how much tissue removed or liposuction performed that does affect post procedure estrogen production.  During menopause, the ovaries produce less estrogen, and fat tissue becomes a more prominent source. This can lead to changes in body composition and fat distribution, as well as other menopausal symptoms.

Q: Can I still get good results even after years of aging and weight fluctuations?

A: Absolutely. While results vary based on your body and health, many women in their 40s see dramatic, rejuvenating outcomes that restore both form and function.  Unfortunately with the rapid weight changes occurring with pregnancy the skin does get stretched out and our available options for skin shrinking are not powerful enough to compete with surgical excision.

Q: What if I had surgery in the past (like breast augmentation)?

A: We frequently revise or update prior procedures as part of a mommy makeover—whether it's lifting previously augmented breasts or tightening lax abdominal muscles.
