Surgeon's Perspective

Dr. Levesque on the Breast Augmentation

A very thin patient with a wide intermammary gap is one of the hardest augmentations — the implant alone will show edges and rippling. Combining a dual-plane implant with fat transfer to the sternum and upper poles softens those transitions and hides the implant.

Scoring the constricted tissue lets it expand and redistribute over the implant. When the tissue stays thin, a planned second stage to fine-tune volume and add more fat is the right call rather than forcing it in one operation.
